Health Records

NKY Health maintains a history of our patients’ health records to help provide a complete picture of their health history.

 

All of our records are now kept on MyChart, a free and secure online portal that allows users to exchange messages with providers, request appointments, renew prescriptions, check test results, and much more.

MyChart also works in conjunction with other health organizations, such as St. Elizabeth, so all of your records are together in one place.

 

Other Health Records

Paternity Acknowledgement

Parents who are not married and who did not have the father’s name listed on the birth certificate at the hospital may want to legally establish paternity by completing a paternity affidavit at the Health Department’s county health centers. (Note: the Health Department does NOT provide paternity testing.)

Birth and Death Certificates

The Health Department does not provide copies of official birth and death certificates locally. They must be obtained from the Kentucky Office of Vital Statistics.

Northern Kentucky residents wishing to obtain an official copy of a birth or death certificate have four options:

1.  Go to one of the Health Department’s county health centers to obtain a birth or death certificate application. The Health Department’s staff can provide help filling out the application, but you are responsible for mailing the completed application.

2.  Call the VitalChek Call Center at 800-241-8322 to order a copy of the birth or death certificate by phone. You must have a credit card to pay for the processing and mailing of the certificate when ordering by phone.

3. Order the birth or death certificate online at www.vitalcheck.com. You must have a credit card to pay for the processing and mailing of the certificate when ordering online.

4. Go to the Kentucky Office of Vital Statistics in Frankfort to request a copy in person.

Please note:  The Kentucky Office of Vital Statistics has specific procedures that must be followed in order to obtain birth or death certificates.  These procedures can change without notice.  Please read their Business Procedure for Accepting Walk-In/Mail/Phone Orders before attempting to obtain any records.

Immunization Records

Residents of Kentucky may a​​​​​​​​​​​​​​​​​​​​​​​​​ccess their or their minor’s official immunization record for free through the secure Kentucky Immunization Registry (KYIR) public portal. Portal users may choose to receive a .pdf copy of their complete records, school certificates for students younger than 18, or COVID-19 vaccine record.
