Reporting A Case

If you are a school or business, please use this form to report employees and/or students who have been diagnosed with COVID-19: https://redcap.research.cchmc.org/surveys/?s=W9H9KTR4PX

If you are a school, don’t forget this form must be completed in addition to the report you submit to the Kentucky Department for Public Health. Here is the link for more information on school reporting: https://chfs.ky.gov/agencies/dph/covid19/K-12%20SchoolSelfReportingPacket.pdf

Please note that the numbers reported by the Kentucky Department for Public Health may not match numbers being reported locally. Local Health Departments only report lab-confirmed cases of KY residents. Schools should be reporting all cases in students and staff regardless of residence.
