We know that when someone is diagnosed with HIV – and they receive and continue medical care – they can lead a long and healthy life.
We’re here to help. Our trained experienced staff can help answer questions and get you on a path toward a healthy life with HIV.

Your health is our goal, and we want to help you achieve that by providing you with the resources and knowledge you need after an HIV diagnosis. Our care coordination staff will discuss available HIV care and services with anyone, regardless if you are a client.
Case Management
The goal of NKY Health’s HIV/AIDS case management services is to provide those diagnosed with HIV and members of their household with a range of care, including support, stabilization, and referrals to community resources.
Our direct services include:
- Assistance with communicating the diagnosis of HIV and necessary referrals for those persons diagnosed. Health Department staff can meet with patients in their provider’s office.
- Case reporting
- Partner notification, partner testing and education
- Short-term counseling
- Help in finding doctors/dentists
- Support groups
- Education about living with HIV/AIDS
- Possible financial assistance with medical, dental, and mental health needs, medications, health insurance premiums, and transportation.
We also work with providers to:
- Provide linkage to HIV-specific medical care including but not limited to:
- Referral to infectious disease providers
- Assistance with enrollment and other social service programs
- Assistance with supportive services such as dental care, housing, transportation, counseling, etc.
Our HIV case management services extend to those people living in Boone, Campbell, Carroll, Grant, Gallatin, Kenton, Owen, and Pendleton counties.
