HIV/AIDS Case Management Program: What to expect at your first appointment

Case management services at the Health Department are for persons who have tested HIV positive, their family and friends. The intake is meant to provide education and information about the program whether you are newly diagnosed or new to the Northern Kentucky area.

Woman meeting with two clientsTo make an appointment, call the intake coordinator, Clint Ibele, at 859.363.2094. He will collect basic information about you and then schedule you for one of the weekly intake appointments, which may be done at our office, at your home, or another location as determined by you and the case manager. Please refer to the checklist of documents you will need to provide at your first appointment.

If you come to our office, you will be greeted by the receptionist and asked to sign in so there is a record of your being in the building. This form is confidential and you need only provide your initials if you wish. You need not state the purpose of your visit other than to say you are here to see (the name of your case manager). All information is protected by the Health Insurance Portability and Accountability Act.

Other resources for services will be given depending on an evaluation of your needs. These will then be written as a care plan with goals and steps to meet those goals. If you decide you want to be in the program, you will complete several other forms, including an informed participation agreement, a release of information and policies to be signed.

The initial intake may last up to two hours and it may take more than one appointment to complete all the necessary forms. Intakes and all forms are to be completed within 30 days of your initial appointment.

Please tell your case manager if you need or have not been to your first medical appointment prior to your intake appointment. Your case manager can provide you with contact information of local providers for this first appointment. Please note that your choice of provider may be determined depending on whether or not you have private insurance, Medicare and/or Medicaid.
