Birth and Death Certificates

The Health Department does not provide copies of official birth and death certificates locally. They must be obtained from the Kentucky Office of Vital Statistics.

Northern Kentucky residents wishing to obtain an official copy of a birth or death certificate have four options:

The Health Department's staff can provide help filling out the application, but you are responsible for mailing the completed application to the Kentucky Office of Vital Statistics in Frankfort.
  2. Call the Kentucky Office of Vital Statistics at 1.877.817.7362 to order a copy of the birth or death certificate by phone. Note that you must have a major credit card to pay for the processing and mailing of the certificate when ordering by phone.
